What is 1:1 support?

This is where following your referral and triage appointment you will be allocated to a life connector who will make time to come and see you to discuss your referral. This could be at your home, or in the local community if you are feeling comfortable. The aim of this initial visit is to start to get to know you and understand what your needs and wants are, and how we can work together to help you achieve your goals.

To help you achieve your goals we need to ‘work together’. We can only do so much as your life connector, so you would be expected to participate and attend fortnightly meetings or catch ups, but we will go through all of this with you when we first meet.

What can I expect from your support?

Your support is tailored to your needs, you will have an individual plan that will stay with you but that we will update on each visit.

You may decide that your main goal is to attend a weekly group, or you may want to initially just get out of the house. Whatever your needs are we will work with you to achieve them.

There are many ways in which we can do this: it could be that you just require information. Or we can help you with gaining confidence to attend a new group, or if you are struggling with the cost of living and finances we can help you make the correct connections.

As mentioned, we ‘work together,’ so it is really important that you actively contribute to your plan for you to get the best possible outcome.
